Waiver

Purdue University requires all F-1 international students, to participate in a University-sponsored medical insurance plan or to obtain an approved waiver of coverage.

You will be able to apply for a waiver during the following enrollment periods:

Fall Semester: 07/01/2025 at 8AM ET - 09/02/2025 at 5PM ET

Spring/Summer Semester: 12/01/2025 at 8AM ET - 02/02/2026 at 5PM ET

Student Health Insurance Plan Waiver Criteria

Purdue University- Fort Wayne has established four circumstances in which an international student can request a waiver to the Student Health Insurance Plan (SHIP) by providing alternative health insurance coverage. To be approved for a waiver, your health plan must meet the requirements and be submitted with proof of documentation prior to the deadline, 09/02/2025 by 5PM ET.

Students must meet one of the four following criteria:

1. Be 100% funded by your home government including tuition, housing and fees, and health insurance that meets Purdue’s waiver requirements below. Governments providing funds for students to purchase another plan in place of Purdue’s do not qualify.
2. Be a J1 exchange student on an approved exchange program.
3. Be covered as an employee or dependent of an employee of a US-based company/ Canadian based employer with a US-based health insurance plan that meets Purdue’s waiver requirements below.

Waiver Requirements:

1. Coverage must be in effect as of the date of entry into the U.S.
2. Medical benefits per accident or illness of at least $200,000. *
3. Repatriation of remains in the amount of $25,000. *
4. Medical evacuation coverage in the amount of $50,000. *
5. An annual deductible per year not to exceed $500* per illness (per person).
6. To qualify, an insurance policy must be underwritten by an insurance corporation with an A.M. Best rating of "A-" or above, an Insurance Solvency International, Ltd. (ISI) rating of "A-I" or above, a Standard and Poor's Claims Paying Ability rating of "A-" or above, or a Weiss Research, Inc. rating of B+ or above. Alternatively, the participant's policy may be backed by the full faith and credit of the government of the exchange visitor's home country.

*Amounts must be in US dollars ($) and are the minimal amounts required at the time a waiver request is reviewed at AHP.

Acquire the following required documents before clicking below to submit your waiver information:

1. An electronic copy of the front and back of your health insurance ID card.
2. An electronic copy of your complete insurance policy document, including exclusions and limitations and dependent coverage.
